Transitioning home after a hospitalization can be stressful and overwhelming. People with terminal illnesses and multiple chronic medical and mental health conditions are most prone to harm from inadequate transitions, especially if they have fragile support systems in the community. CareNet, Inc. nurses and caregivers work together to make this transition as smooth as possible.
CareNet supports both the hospital and our clients every step of the way, providing care coordination and patient education that ensures comfort and safety. Our process builds patient confidence in managing care at home.
Our post-hospital services include:
- After-surgery care for hip and knee surgery
- Oncology care for clients receiving chemo or radiation treatments

Rehabilitation assistance after a lengthy hospital stay helps improve independence at home and will reduce the chances of readmission. There is no delay in care at discharge. Same day services are available for the transition home if needed.
